so the other day I did a big tow job. somewhere I lost my tailpipe extension. I was pretty sure I lost it Tuesday when I took the trailer back to the nesting place. So I welded on an extension which was too short last night, and then welded on another that I shaped a bit on the end. Today, I went to a buddy's house. On the way home, on the same return route, there my extension was laying on the side of the highway.

Second time on the same wheel I have not been driving the truck lately and nobody checks to make sure the lugs are tight
Think it might be the rims flexing too much

Try some lock tight on the threads. Why I don't use after market rims
