Draw straw sucking air is really common on the 98-02 trucks. Usually it does it around the half tank mark.

Could be. Could also be a bad cam position sensor. A dtc would be nice I'm sure he has to be getting one.
Also check your fuel return lines as they are not under much pressure and rarely show a big leak. What may look like an oil leak may actually be a fuel leak. Also check your fuel injector lines to make sure they are lined up properly. Do you smell fuel while idling or under heavy throttling?
This is a common problem with the vp44 trucks. There is a fuel overflow valve on the return side of the vp44 injection pumps that wear out over time and will help cause hard start issues
