Also play with settings on that Smarty. I've known some guys to get more power out of their trucks on a lower setting than on the higher ones. I believe one buddy has best luck with sw7 on his 02.
Iirc he saw 430 something on sw7 and manual trans, otherwise mods pretty close to yours.
