So I didn't get a complete change in color on the test fluid, but it did become a slight yellow-green color.

I've never done this test personally, but I do recall the color should never change, and bubbles concern me.
Assuming gases are feeding into the rad, which is where the color and bubbles are being generated from.
The hard upper hose and reservoir pressure tell me it has a leak on the cylinders.

I've never done this test, either.
All the symptoms it's exhibiting make me think it's started to go.
Is anyone that has done this that can chyme in?

Just curious, you mentioned your top radiator hose is hard, and you have no heat, is your thermostat possibly stuck closed?

These were my thoughts as well.
After he mentioned he got the thermostat to cycle, I figured it was good.
However, just because it opens occasionally doesn't mean it's good.
I would try a new thermostat to eliminate the possibility of cheap reassurance.
You have the typical mileage "like mine," whereas if it's the OE HG, it will give you and the wife a piece of mind doing the rebuild.
Another thing to think about is the Transfer case.
From a bit of time researching, the chain will start to stretch anywhere from 160k to 240k miles.
I'm in that boat right now.
I am keeping up with the recommendation for fluid changes.
I let this one slip, I think.

When it started having issues, I replaced the temp sensor, the thermostat, and the radiator cap.
As I thought all the same that those parts had just worn out, but the symptoms continued.

I just replaced it when the issues started.
I thought the same thing.
It didn't resolve any of the issues.

Based on the combustion test results with bubbles and slight chemical color change, it’s likely the head gasket.
You could also check how your engine oil looks and any signs of coolant contamination or increased level.
You could pressurize your coolant system and check for leak down, or if you have access to a pressure transducer and lab scope, look for pressure pulses with the engine running.
