Ahh, ok. Surging when in gear. Maybe the turbo needs to be cleaned. The veins might not be fully disengaging while cold.

A bad FCA valve on CP3 pump can make it surge.

Is this a 2010-12 model truck? If so, is the throttle valve unplugged?

Throttle valve deleted

Then most likely it is either exhaust back pressure sensor problems or turbo actuator problems.

I was thinking back pressure as well or turbo as well. Not sure if he has a flow through exhaust though.

So maybe a through cleaning of the turbo

Won't hurt. I'd put a new exhaust back pressure sensor on it and clean out the pressure tube feeding the sensor as well. Unlike the 13up trucks, the 10-12 trucks need a properly working exhaust back pressure sensor for the turbo to work correctly.

The actuator is an exterior component of the turbo. It may not be actuating properly.
